Inter-Facility Patient Handover Report - June 25, 2018

As many of staff may have heard, there is a provincial working group looking at streamlining, standardizing and generally improving the process for moving patients around the province to access care.  

One of the first products that has come out of the group is a refresh of the nursing standard for ensuring appropriate communication of patient information at points of transition. 

Effective July 3, 2018, PTN staff will not be providing ‘report’ to receiving nurses on behalf of sending nurses.  We need to ensure that there is a direct nurse to nurse handover prior to the patient being picked up for transfer to another site.  

PTN staff will provide contact information for the receiving site in order to facilitate this process. In this context, the term "nurse" refers to the LPN/RN/RPN who is or will be the most responsible nurse for the patient.
