Complex Discharge Rounds

Weekly rounds with representation from our Community team and clinical leadership from all levels occur when the needs of our patients with long stays and complex discharge needs are discussed. 

These discussions are chaired by our Complex Care Navigator. 

Our Medicine, Surgery and Mental Health programs have all adopted unit-based rounds. 

An emphasis is now being placed on ensuring that everyone understands what escalation is and what the process is to do this.
