Root Cause Analysis (Incident Analysis)

In healthcare, patient safety incidents that impact the lives of patients and families, as well as providers and organizations, can and do occur. The Canadian Incident Analysis Framework is a resource to support healthcare providers as they manage, analyze, and learn from patient safety incidents, with the goal of enhancing the safety and quality of patient care. 

PHC offers a full-day workshop to: 

• Review the impact of system factors on error potential
• Provide an overview of the application of human factors engineering principles in health care environments
• Describe the evolution and components of incident analysis
• Simulate an incident analysis of a critical incident
