Get Vouchers

Taxi vouchers are ordered through PHC Finance by printing, filling out, and faxing in the below taxi request form to 604-806-9085.

Black Top Taxi Voucher Request Form

There is no cost associated with ordering vouchers - they come in books of 50 and are individually stamped with a unique number. The vouchers can be picked up in person or couriered to your department at your cost. The courier will be charged to the cost center that ordered the taxi vouchers.  

Do Not Share Vouchers - each department using taxi services should have their own vouchers.

Taxi services can be booked by calling the Health Authority's special phone line for Black Top Taxi, listed above. Each voucher is good for the payment of one trip; therefore, to travel to an appointment and return at a later time, you will require two vouchers.

Using Taxi Vouchers

Log the taxi voucher in the Custodian Log book.
