Patient Safety Learning System (PSLS)

The BC Patient Safety & Learning System (PSLS) is a web-based tool used by healthcare professionals across British Columbia to report and learn from adverse events, good catches (near misses) and hazards that occur in healthcare settings.

PHC’s culture of safety strongly encourages staff to report adverse events and “near misses” or close calls.  Our non-punitive policy is designed to encourage all staff to feel they are partners in improving safety of the patient/resident care processes and systems.

Managers, leaders and directors are responsible for reviewing details on the reports, including a review to ensure that the information entered on the form is complete and accurate. PSLS reviews are to be initiated within 5 days of the report being completed.
