We have been working together to implement some significant changes to the way we transition our patients and residents from admission to discharge across the organization to maximize our patients' and residents' access to appropriate care.
Flow Facts have been created as an efficient and succint way share flow updates, success stories and definitions with the staff and leaders involved, and to connect this daily work to PHC's larger organizational goal of being Quality-Forward our new 7-year strategic plan, Mission: Forward.
